| This course provides awareness training for FAA managers and supervisors on the FAAs Safety Management Information System (SMIS) used to report employee work-related accidents, incidents, and mishaps. Training topics include the purpose of SMIS, completion of FAA Form 3900-6 (Mishap Report), re-cordable criteria, generation of OSHAs Form 300 (Log of Work-Related Injury and Illness), and OSHAs Form 300A (Summary of Work-Related Injury and Illnesses).
