This course is designed to provide instruction in the theory, techniques, and application of root cause analysis. An effective root cause analysis is designed to address problems in a structured way, with the intent of preventing recurrence of the problem. A successful root cause analysis, with an emphasis on causal factors, contributes to effective problem solving and sustaining corrective actions. Students will be provided with the tools to lead a root cause analysis session with team members in their organization. The course will benefit auditors, safety managers, reliability analysts and investigators at the entry level as well as those who are familiar and want to enhance or refresh their skills in the practice of root cause analysis. The course is also valuable to organizations who have implemented a Safety Management System (SMS), a Continuing Analysis and Surveillance System (CASS) or an Internal Evaluation Program.
